JOB SUMMARY / PURPOSE
The Diabetes Educator assists the nursing staff with the education of patients with diabetes and provides education for the nursing staff to maintain current knowledge and skill in the area of diabetes.
Serves as a resource for diabetes within the hospital setting.
ESSENTIAL KEY JOB RESPONSIBILITIES 1.
Assesses patient education needs and provides teaching for patients (and families/care givers) with diabetes
Provides initial and follow-up consultations (as needed) for patients requiring education related to insulin administration and blood glucose testing
Teaches diabetes disease process and treatment including diet, medication action, and complication prevention
Assesses patients on insulin pumps and educates as needed; reviews and documents pump settings, and troubleshoots pump functions if needed
Initiates referrals for outpatient diabetes education services
Documents in the medical record and communicates with health team members (physicians, nurses, case managers) regarding patient education and outcomes
Participates in departmental continuous quality improvement projects
Participates in development and implementation of policies and procedures related to: JCAHO, the American Diabetes Association, and the American Association of Clinical Endocrinology standards
Maintains Certified Diabetes Educator (CDE) status or 15 hours continuing education annually in diabetes
Other duties as assigned by management
BSN; graduate of accredited school of nursing
Required Licensure and Certifications:
RN licensed in state of TN; BLS Required
Minimum Knowledge, Skills and Abilities:
3 years RN experience; Basic Word and Excel computer skills
Certified Diabetes Educator (CDE); experience in diabetes and/or patient education
