Family Caregiver Support Specialist

Responsibilities include case management and advocacy for family caregivers of older adults, persons with dementia and non-birth children living within a 10-county service area, program eligibility determination, conducting in-home assessments, developing and coordinating individualized service plans, planning and facilitating support groups and organizing educational and enrichment opportunities. The ideal candidate will have excellent communication skills (both written and verbal), be able to maintain confidential information and work independently with minimal supervision or as part of a team. Candidates must possess outstanding organizational and computer skills and be able to establish and maintain positive and effective working relationships with internal/external contacts and partners. Must have a bachelor’s degree in a human or social service field. Candidates with the ability to effectively determine the needs of relative caregivers and match those needs with appropriate services as demonstrated by training or experience may be considered.
