National Family Caregiver Support Program Coordinator
Southeast Tennessee Development District Area on Aging & Disability Department National Family Caregiver Support Program Coordinator The Southeast Tennessee Area Agency on Aging & Disability is seeking a Coordinator for the National Family Caregiver Support Program (NFCSP). Responsibilities include screening caregivers for eligibility, conducting comprehensive in-home assessments of caregiver needs, developing individualized care plans, coordinating services, facilitating caregiver support groups and coordinating caregiver education and training within a 10-county region. The ideal candidate will have excellent communication skills (both written and verbal), be able to maintain confidential information, and have outstanding organizational and computer skills. Candidates must be able to establish and maintain positive and effective working relationships with internal/external contacts and partners, be able to work independently with minimal supervision and as part of a team. A Bachelor’s Degree in social work or a related field is required.
